I hereby release U-GRO Learning Centres, Inc. from any/all liability of whatever kind of nature which, at any time, could result from obtaining and having an employment decision based on such information.

Employment or volunteer service in a licensed child care center is conditional upon a background check. U-GRO Learning Centres, Inc. is licensed by the Department of Public Welfare and complies with its requirements to perform necessary background checks and drug use testing on all employees. As a condition of employment, I understand that I must meet these requirements.
